Please sign the volunteer liability waiver to get started: I wish to provide volunteer services to Haven to Home, Inc. Canine Rescue. ("H2H"). H2H is a non-profit, 501(c)(3) organization, which serves as an animal humane society. My services will involve assisting H2H with a variety of rescue, rehab, transporting, community events and other related volunteer needs. I understand that H2H is not familiar with the full background of all animals that come into its rescue, care and custody. Animals may have vicious tendencies that are unknown to H2H. I also understand that the behavior of animals is not predictable. I understand that there are many risks involved in volunteering with animals. I represent that I am familiar with those risks, such as animal bites and scratches, transfer of disease or illness, and injuries from erratic behavior such as jumping on individuals or otherwise being unable to be controlled. I am voluntarily assuming the risk associated with my activities as a volunteer for H2H. In addition, I release H2H from all liability for injuries and damages to me and to any of my property that occurs during or results from my acitivities as a volunteer for H2H, unless the injuries or damage result soley from the willful and intentional actions of H2H or soley from H2H's gross negligence. This release of H2H includes all of its officers, agents, employees and representatives. I understand that H2H will rely on this Release of Liability in accepting my offer to provide volunteer services to H2H.
